When you arrive for your first visit, please arrive about 15 minutes early so that we can get you acquainted with the office and our staff.
To save time during your first visit, you may download a medical history and financial policy form from this website ahead of time, fill them out, and bring them with you – otherwise you will need to complete them when you arrive. Or if you prefer, you can also fill out these forms on-line and avoid having to fill them out at the office.
